Request it from your insurance agent and list “CaptainGreek Marketplace” as certificate holder.

Voided company check or bank letter

Links your business bank account for payouts.

Routing and account numbers must be visible so payout details can be verified.

Confirms the identity of the primary contact.

Upload the front and back separately if the application asks for both sides.
