Before you begin
Uploads allowed: pdf, jpg, jpeg, png.
Business & Account
- Business name, type, and registration State.
- Tax Identification Number (EIN).
- Form W‑9 and, if applicable, your Business License.
- Government‑issued Photo ID (passport or driver’s license).
Address
- Business address details: Company, Phone, Street, ZIP/Postal Code, City, State, Country.
Banking & Payment
- Voided Check for ACH payouts verification. We don’t collect bank numbers on this page.
Insurance
- ACORD 25 — Certificate of Liability Insurance.
- Merge the ACORD 25 and all endorsements into one (1) PDF before uploading.
- Policy must include Additional Insured (primary & non‑contributory) and Waiver of Subrogation for CaptainGreek Group of Companies, LLC.
Optional (if applicable)
- FDA food compliance, Alcohol license, Precious‑metals certification, Certificate of Origin, or Customs documentation.
After our team reviews your documents, we permanently delete the uploaded files from our servers.
We may retain an offline copy (physical or encrypted digital) only as required by law and in accordance
with our Privacy Policy.
